FDA backs Vytorin for kidney disease patients

WASHINGTON (AP) ? The Food and Drug Administration says Merck's cholesterol drug Vytorin helps reduce heart attack, stroke and related problems in patients with kidney disease, a potential new use for the blockbuster drug.

The agency's online review also found no new safety concerns with Vytorin, which came under scrutiny for potential cancer risks in 2008.

Merck has asked the FDA to approve the drug for a new use in reducing heart-related problems chronic kidney disease patients. The combination pill is already cleared to reduce bad cholesterol.

FDA says that Vytorin lowered kidney disease patients' heart-related problems by 16 percent compared with placebo. However results varied depending on whether patients were receiving dialysis, standard treatment for patients with late-stage kidney disease who can no longer remove waste from their blood. Patients on dialysis only saw a six percent drop in heart problems, compared with a 22 percent reduction for healthier patients not on dialysis.

On Wednesday, the FDA will ask non-government advisers to comment on the disparity and to vote on the overall safety and efficacy of the drug for kidney disease patients.

Vytorin, a $2 billion-a-year drug for Merck, combines two brand-name cholesterol pills Zocor and Zetia.

In July 2008, preliminary results from a four-year study indicated a possible increased risk of cancer in patients getting Vytorin. But FDA's latest review found no evidence of a link between Vytorin and cancer.

"We believe these data should help lay to rest investor concerns about Vytorin and Zetia's safety," Leerink Swann analyst Seamus Fernandez wrote in a research note Monday.

Shares of Merck & Co. Inc. fell 23 cents to $34.88 in morning trading Monday.

US home video spending up for 1st time since 2008 (AP)

LOS ANGELES ? Americans' spending on home videos has finally emerged from the recession ? helped by more purchases of higher-priced Blu-ray discs and greater outlays on cut-rate rentals from Netflix and Redbox.

For the three months through September, home movie spending rose nearly 5 percent from a year earlier to $3.9 billion, the first increase since early 2008, according to industry organization, The Digital Entertainment Group.

Buying digital copies of movies and ordering them from set-top box video-on-demand services also rose.

People bought fewer DVDs and made fewer trips to brick-and-mortar video rental stores, cutting into the gains.

For the year overall spending is down about 2 percent at $12.3 billion.

The industry is struggling to cope with a weak economic recovery and the gradual wane of the DVD era. The digital discs, once revolutionary for their clarity and durability, were shown the exit once Blu-ray became the industry's high-definition standard in 2008. Compared to a year ago, DVD sales dropped by 15 percent, or about $230 million, to $1.32 billion.

Blu-ray disc sales rose by about $156 million, to $423 million. That didn't make up for the DVD drop, even with the help of the popular re-release of the six "Star Wars" movies on Blu-ray, which racked up $38 million in North American in its first week of sales in late September.

There's still room for Blu-ray to grow. Although Blu-ray player households rose by 52 percent to 33.5 million, that's still only about a third of the TV-owning homes in the U.S.

Brick-and-mortar store rentals fell by $142 million to $353 million, but they were more than replaced by a $152 million gain in new delivery methods, such as online streaming, video-on-demand, mail-order subscriptions and cheap rentals from kiosks. Revenue from those rental methods rose to $1.70 billion. Kiosk rentals made up more than half of those gains.

Netflix Inc.'s price hike to customers on Sept. 1 may have bolstered the numbers, and Redbox said it was raising its nightly DVD rental fee to $1.20 from $1, which will likely add to the current quarter's totals.

The smallest segment remains digital purchases of movies, which rose by about $15 million to $136 million. Movie studios have been concerned that people aren't purchasing digital movies because they are worried the files won't be easily transferable to various devices, a concern it hopes to ease with its UltraViolet view-anywhere standard, which Warner Bros. launched this month.

New drug shows promise against multiple sclerosis

ScienceDaily (Nov. 1, 2011) ? An experimental drug called Ocrelizumab has shown promise in a Phase 2 clinical trial involving 220 people with multiple sclerosis (MS), an often debilitating, chronic autoimmune disease that affects an increasing number of people in North America. It usually strikes young adults and is more common in women than in men.

The study, carried out by researchers at UCSF Medical Center, and involving hospitals in the United States, Canada, and Europe, is described this week in the British medical journal The Lancet.

The study involved patients with relapsing-remitting MS, a form of the disease marked by the accumulation of lesions in the brain and spinal cord and periodic "attacks" of neurological impairment.

The 220 patients were randomly enrolled into four groups -- two that received injections of the monoclonal antibody Ocrelizumab at two different doses, one that received the standard multiple sclerosis drug interferon-beta, and one "control" group that was given a placebo.

The doctors gauged the effectiveness of each treatment by performing monthly magnetic resonance imaging (MRI) brain scans of the patients and counting the number of visible marks that indicate inflamed lesions, a hallmark of the disease. They also compared the severity and frequency of neurological "attacks" that cause loss of vision, incoordination, weakness and numbness, among other symptoms.

The results of this trial showed that patients who received the drug generally fared well and showed fewer signs of the disease than patients who receive a placebo or the standard Interferon treatment. Overall, the trial found that Ocrelizumab led to a 89 percent reduction in the formation of brain lesions, and it also reduced the number of new multiple sclerosis attacks over 24 weeks. During this relatively short-term study, interferon performed no better than placebo on these outcomes.

"It really is a remarkable finding," said Stephen Hauser, MD, the Robert A. Fishman Distinguished Professor and chair of the Department of Neurology at UCSF who was the senior author on the study. "This is extremely exciting, both in terms of the prospect of improved therapy for people with multiple sclerosis but also for the lessons that it teaches us about the fundamental cause of the disease."

"The prospect of an extremely effective therapy for multiple sclerosis that can be safely administered at six month intervals could represent a major step forward, if the safety profile and benefits are sustained over longer periods of use," he added.

At 24 weeks, serious adverse events were reported in 4 percent of patients in the placebo arm, 4 percent of those taking interferon, and 2 percent and 6 percent of patients taking 600 mg and 2000 mg of Ocrelizumab. One patient taking Ocrelizumab died, but the relationship with the study drug, if any, is as yet unclear.

Hauser said that the next step for the drug will be to see if the drug's effect and positive safety profile will be sustained over time. These questions will be addressed in two parallel Phase 3 trials, now enrolling a larger number of patients who will receive the drug regimens for a longer period of time. This trial is now underway at several hundred sites around the world. Hauser serves as lead investigator for these trials, which are sponsored by Roche, the company that owns Ocrelizumab.

What the Trial Suggests About Multiple Sclerosis

Besides the obvious question of whether the drug would work, also under scrutiny in the clinical trial was whether a drug like Ocrelizumab would work in the first place. Its mechanism of action is fundamentally different than other existing therapies for multiple sclerosis. All multiple sclerosis drugs work by targeting a person's immune system -- only they target fundamentally different parts.

In multiple sclerosis, a person's immune system attacks the myelin sheath that insulated nerve fibers in the brain and spinal cord. Most of these attacks have no associated symptoms, but damage to these sheaths can short circuit signals traveling along the nerve fibers. This disrupts the normal flow of communication from the brain and can cause a range of symptoms -- including visual impairment, weakness, sensory disturbance, fatigue, cognitive difficulties, and loss of coordination.

For decades, work in the field has focused on targeting one component of the immune system -- the T cells -- which were always seen as the "smoking gun" of the disease because they attack nerve fibers in the brain, causing the lesions and ultimately the symptoms of multiple sclerosis. Ocrelizumab, in contrast, targets a molecule called CD20 found on the surface of B cells, a separate component of the immune system.

Although the mechanism is not exactly clear, said Hauser, the B cells seem to induce the T cells to attack. If T cells are pulling the trigger on nerve fibers in the brain, B cells seem to be the ones that are loading the gun. The Phase 2 trial showed, in essence, that if you block one, you may be able to stop the other.

Hauser said the recently concluded trial validated the idea that targeting the B cells could ultimately help people with multiple sclerosis -- an idea that Hauser has championed for more than a decade after basic research in his laboratory in the 1990s suggested that these cells play a role in the disease.

"This is a great example of lessons learned at the bench bringing us to an experiment at the bedside that completely transformed our understanding of what we needed to do to develop more selective and more effective therapy for MS," he said.

Egypt Mubarak's trial to be postponed again - lawyers (Reuters)

Cairo (Reuters) ? The trial of Egypt's ousted President Hosni Mubarak, which was due to resume on Sunday, will be postponed until a decision is made on whether to change the panel presiding over the case, said lawyers on Saturday.

Some of the lawyers representing families of those killed in an uprising against Mubarak's rule have demanded changes to the panel after complaining it had failed to give them adequate time to question Field Marshal Mohamed Hussein Tantawi, who heads the army council now ruling Egypt.

Lawyers told Reuters the current panel headed by Judge Ahmed Refaat will either hold a session to adjourn the trial while remanding the defendants in prison or will postpone it through administrative methods without holding the session.

"The case is suspended until a decision is reached on Refaat," said Hassan Abul Enen, a lawyer representing the families.

The delay will add to public frustration over the pace of the trial.

Al-Shorouk newspaper reported on Saturday the three judges appointed to decide on the lawyers' request to change the panel in the Mubarak trial had resigned.

The head of the cassation court now has to appoint a new team of judges who will rule on the lawyers' demand, a move likely to further delay the trial of the former president.

The panel had previously postponed hearing the demand until December, saying it needed to review Refaat's record and background, specifically requesting details of any consultancy positions he was involved in.

Judicial sources told Reuters on Saturday the Supreme Judiciary Council, the highest judicial authority in Egypt, said in a report that Refaat has not been appointed in any governmental positions since 1998.

Egyptian judges are often used by government ministries or bodies, a practice critics say has often brought the objectivity of some judges into doubt.

Refaat has a reputation of working by the book and following procedures. He has been praised for his independence by the media.
